Dental Plan Information

 You have TWO Plans to choose from: 

The DIFFERENCE between the two plans is the SILVER plan offers a 50% reimbursement schedule and only a 30 day waiting period for TYPE 1 and TYPE 2 Treatments and is therefore less expensive than the Bronze plan.

The Bronze plan offers the more traditional reimbursement schedule of 80% for TYPE 1 - Preventive/Diagnostic Treatment with a 30 day waiting period, 80% for TYPE 2 - Restorative Treatment with a 6 month waiting period, and 50% for TYPE 3 - Major Treatment with a 1 year waiting period. Both plans offer an Orthodontic benefit after a two year waiting period.

 DENTAL SILVER PLAN - A [50% - 50% - 50%] Plan

  • Type 1 - 50% Reimbursement for Preventive Diagnostics (Fluoride Treatments, X-rays, Cleaning, Periodic Exams)

  • Type 2 - 50% Reimbursement for Basic Restorative (Extractions, Filings, Oral Surgery)

  • Type 3 - 50% Reimbursement for Major Restorative services (Root Canals, Bridges, Crowns, Dentures,Partials, & Periodontics)

  • Type 4 - 50% Reimbursement to $1000 for an Orthodontic benefit (for ages 6 to18) after a 2 year waiting period

Please be advised their is a 30 day waiting period after the 1st of the month for coverage to begin. The 1 year waiting period applies to only Major Restorative services and a 2 year waiting period applies to only the $1000 Orthodontic benefit (for ages 6 to 18).

Plan Questions and Answers

1. What are Preventive/Diagnostic Services?

Exams (up to 2 exams per year)

Prophylaxis (cleaning)

Fluoride treatments

X-rays

Benefits are paid at 80% under the Bronze Plan and at 50% under the Silver Plan (initial 30 day waiting period applies for either plan).

2. What are Basic Restorative Services?

Extractions, Fillings, & Oral Surgery

Benefits are paid at 60% (after 180 day waiting period) under the Bronze Plan and 50% (after only a 30 day waiting period) under the Silver Plan.

3. What are Major Services?

Root Canals, Bridges, Crowns, Dentures, Partials, & Periodontics. Benefits are paid at 50% (after 365 day waiting period) for the Silver Plan ONLY.

4. Are Orthodontic benefits covered under the program?

Yes! Orthodontic Benefits are paid @ 50% for dependent children ages 6 to 18 and covered by the policy. Maximum Benefit is a $1000 Lifetime amount per child. Example: If Orthodontic Costs are $2,200, 50% would be paid to a maximum of $1,000 (available after a two year waiting period). Silver Plan ONLY.

5. When does this policy expire?

Your dental policy does not terminate at the end of a policy period. It will continue as long as you want to continue the coverage and the premiums and association fees are paid.

6. How long may I keep my coverage?

You may keep your coverage as long as you pay your premiums and association fees, and the Master Group Policy No. 9811 sponsored by the NRAA, remains in force.

7. May a member switch coverage from the Silver Plan to the Bronze Plan?

YES, but only on the member's policy anniversary date.

8. If a member chooses to switch to a different plan does a new waiver period need to be satisfied?

YES, since the new plan has a different premium pricing structure the waiver periods must be satisfied. Once a member has received benefit services paid by the current plan, they may be in a better position to get through the waiting period of the new plan.

9. If a member receives services out of the country, e.g., Canada, will benefits be paid in accordance with the policy?

YES, benefits will be paid at the highest R&C (reasonable & customary) level (Level 9) for all international services.

10. Why is it necessary for me to join the NRAA (National Representatives Association of America)?

NRAA is a group Association Dental Plan offering all the traditional group benefits. All group plans require a common relationship to exist between plan sponsor and the insured's. In most group plans it's an employer/employee relationship. In this case, since members are individuals and not employees, they need to join the NRAA who sponsors this group program. As a member of the larger (common) group they are then eligible to participate in the Dental Care Plan. The Association Fee is $2.00 per month and joining the NRAA is a seamless process when applying for the NRAA Dental Program at the same time.

11. What forms of payment are considered acceptable?

Members may apply Online by clicking on the 'Quote' link, over the telephone by calling 800-286-6244, or by mail with a VISA/MC/DISCOVER debit or credit card. Checking account ATM cards with the MC/VISA logo are also accepted and the premium would be deducted directly from their checking account.

Members may also authorize a monthly Bank Draft providing Universal Health Systems authorization to deduct the monthly premium from their account electronically. Sending Money Orders and other monthly payments directly are unacceptable forms of payment.
